Standing on Common Ground: Monroe County Children's System of Care

A System of Care for children and youth with mental health challenges

“In a system of care, mental health, education, child welfare, juvenile justice, and other agencies work together to ensure that children with mental, emotional, and behavioral problems and their families have access to the services and supports they need to succeed.”
 
“Systems of care are developed on the premise that the mental health needs of children, adolescents, and their families can be met within their home, school, and community environments. These systems are also developed around the principles of being child-centered, family-driven, strength-based, and culturally competent and involving interagency collaboration.”
 
In Monroe County, the Office of Mental Health through the Monroe County ACCESS initiative  is working with its partners to create and sustain improvements in children’s mental health that are innovative, collaborative and proven to have better outcomes for our children and their families.
 
For example, Monroe County ACCESS is facilitating practice change by training Care Coordinators from the Supportive and Intensive Case Management Program, The Home and Community Based Waiver Programs and Youth and Family Partnership in the Child and Family Team  process. It’s a process that embodies the System of Care values  of “family driven and youth guided” in its approach to planning, implementation and evaluation of the youth’s service plan.
