Receive quarterly updates, articles and information on the System of Care in Monroe County.
Being a System of Care community that receives federal funding from SAMHSA, our youth and families, who are receiving Care Coordination services, have an opportunity to enroll in the national evaluation. Every funded system of care community participates in the national evaluation.
The national evaluation provides information about our local community's program achievements, child and family outcomes, and mental health service needs. The evaluation seeks to answer such questions as:
What works in our system of care community?
What are the outcomes for children and families who participate in our program, Care Coordination?
Children's Institute is Monroe County ACCESS's partner in implementing the national evaluation in our community. Interviewers are trained family members who administer the questionnaire to the youth, family member/caregiver and mental health provider.
A series of booklets are available for download (see below) and provide initial data on youth and families who are enrolled in the national evaluation. The booklets take a look at questions that members of the community have said that they would like answered.
Please keep in mind that the information reported in the booklets is preliminary. These booklets give the community a peek at what MAY be happening for youth and families at various times and at specific points in time during the evaluation process.
Although this preliminary data can be helpful in understanding how youth and families appear to be doing, no conclusions can be made yet about “what works” or how youth and families are doing for certain.
These booklets have been presented to several Monroe County ACCESS partners, and have sparked much discussion among families, youth, and providers about their individual experiences with Care Coordination. The groups have also provided feedback on how the information presented in the booklets can be useful to them and they have offered ideas on what other information might be helpful.
